Cancer Coach - terms and conditions.

Cancer Coach is available to inforce Fidelity Life ‘Group Trauma Insurance’ cover customers who, regardless of whether your claim is in progress or has been denied:

  • are noted as the life insured in the policy schedule; and
  • who have a current cancer diagnosis from a New Zealand registered medical practitioner.

Your enrolment in Cancer Coach is, however, separate from your claim and does not mean you need to have a valid claim under your policy.

Cancer Coach is not available on any other Fidelity Life Group covers (e.g., Group Life Insurance), or Direct Covers.

You must be at least 18 years old to use Cancer Coach.

Although Osara Health will be your relationship provider for this support service once you have enrolled in Cancer Coach, the relationship for your policy and your claim remains with us.

Cancer Coach is not a policy benefit and Fidelity Life reserves the right to discontinue it or amend the eligibility criteria at any time. Should you have already commenced participation in Cancer Coach, however, and we elect to suspend or discontinue it, we will work together in good faith with Osara Health to ensure that you will continue to be supported until the conclusion of the service.

Fidelity Life collects, holds and uses your personal information in accordance with the Privacy Act 2020 and our Privacy Statement. You have the right to request access to, and correction of, any personal information held by Fidelity Life.

For customers enrolled in Cancer Coach, your first name, last name, date of birth, email address, phone number and policy number will be disclosed to Osara Health as our third-party service provider in accordance with the terms of our Privacy Statement, and you consent to the use of your personal information for this purpose.

Except for liability that may not be excluded by law, Fidelity Life will not be liable for any loss, claim, cost, expense, liability or injury suffered by you in relation to the provision of Osara Health’s service. You should always consult your treating doctor on any assessment.
